Are you currently being prescribed the contraceptive pill?

We have developed a system whereby, as long as you are well on the contraceptive pill and you provide us with the information that we require, we can carry out  a medication review without the need for you to have a pill check appointment.

(Please note that this process is only applicable to patients aged 18 and over, any patients aged under 18 being prescribed the contraceptive pill will still need to attend face to face review appointments.)

  1. Please check your blood pressure, height and weight using the machines in the waiting room and give the results to one of our receptionists.

Please note that this form is only for the use of women 18 years and over. In order to provide the contraceptive pill safely we need to ask you a number of questions. We would be grateful if you could complete this form when you submit your next repeat prescription request. If you are having any problems with your medication or would like to consider alternative contraception options, please speak to one of our GPs or Women’s Health Nurses, who will be able to advise you.

Please read the following information before completing the checklist:

  • The contraceptive pill may not work if you have diarrhoea or vomiting.
  • Commonly used antibiotics don’t affect the efficacy of the contraceptive pill.
  • Medicines such as some of those used to treat epilepsy, HIV and TB, and the complementary medicine St John’s Wort, may make it less effective.

Your prescription will be processed within 2 working days. If we have any problems with re-issuing your prescription we will contact you. 

Please remember to attend for your cervical smear testing (EVERY 3 years for women aged 25-50 and EVERY 5 years for women aged 50- 65)
